Metro Manila LGUs launch online COVID-19 vaccination registrations

Cities in Metro Manila have started preparing for the expected rollout of Covid-19 vaccines by launching their respective online registration platforms.

The common items in the registration forms include the address, identification card numbers, medical histories, and previous diagnosis for COVID-19 if any, of the interested applicant.

EAGER to ensure an effective and successful rollout of Covid-19 vaccines, several local government units (LGUs) in Metro Manila have opted to use the digital platform for their online registration and schedule.

The move was in anticipation of the expected arrival of vaccines in the coming months following assurances from the national government as well as the delivery of their respective orders through the so-called tripartite agreements.

The common items in the registration forms include the address, identification card numbers, medical histories, and previous diagnosis for COVID-19, if any of the interested applicant.

To register, Makati City residents can visit Makati COVID-19 Vaccination .

To register, upon visiting the website, you will be asked about the beneficiary sector you belong to and as well as your ID number.

Provide the required details to complete the registration.

For the City of Manila, the applicant can check out the Manila COVID-19 Vaccine and click the “register or reserve” button.

Fill up the registration form and click “validate,” enter the one-time password that will be sent through your cell phone number, and click register.

Then click the “I agree” button and affix your signature on the waiver page.

Other cities that also launched their online registrations are Malabon and Las Pinas.

Malabon City residents who want to register for the vaccination program may do so by filling up the details required in a google form from the LGU.

Registrants will be asked to indicate the sector they belong to, PhilHealth number, age, address, medical history, and other personal details. Click “submit” after completing the form.

Las Piñas City residents can also check their city’s link to access the Google form for the vaccination registration.

After clicking the link, you will be asked to identify the sector you belong to, contact number, address, medical history, COVID-19 infection history, and other details.

Registrants will likewise be asked for a 1×1 picture in white background and a photo of your government-issued ID. Click “submit” after filling out all the required fields.

Aside from them, also launching online registrations are Muntinlupa, Navotas, Parañaque, San Juan, Valenzuela, Pasig, Caloocan, Quezon, Taguig and Mandaluyong Cities.
